Terms of service

The service

RecordsApp requests your medical records from providers on your behalf, using the authorization you sign, and organizes them for you. We don't give medical advice.

Your part

Give true information about yourself, and only request records you have the right to get (your own, or as a personal representative). Keep your sign-in to yourself.

Fees

RecordsApp is free at launch. We don't charge you, and we never ask for a card. If a provider asks for a copy fee, we tell you first and pay nothing without your yes. If we ever add a price, we will tell you before it applies.

Limits

Providers decide how and when they respond; we follow up but can't guarantee every record arrives. The service is provided as is. To the extent the law allows, Disrupt Ventures LLC is not liable for indirect or consequential losses, and its total liability is limited to the amount you paid us in the last twelve months.

Electronic signature

You agree to receive notices and sign documents electronically.

Ending

You can close your account at any time. We may suspend an account that breaks these terms.

Changes and law

We may update these terms and will post the new version here with its date. These terms are governed by the laws of the state where Disrupt Ventures LLC is organized.
