The HIPAA release you sign
One authorization lets us ask each of your providers for a copy of your records, for you. Here is what it says before you sign it.
Effective November 3, 2026. RecordsApp is operated by Disrupt Ventures LLC ("we", "us").
Who it lets us ask
Only the doctors, practices, hospitals, labs and pharmacies you add to your list. You can add or remove providers before we send anything.
What it asks for
A copy of your records: visit notes, test results, imaging reports, medication lists and bills. You can leave out sensitive categories (for example psychotherapy notes, substance use treatment records, HIV or genetic test results); those are only included if you tick them.
Who receives the records
RecordsApp, operated by Disrupt Ventures LLC, acting at your direction, so we can organize them and show them to you. Under HIPAA this is your right of access (45 CFR 164.524): the provider sends your records to the person or service you name.
Why
At your request, for your own use.
How long it lasts
One year from the day you sign, unless you end it sooner.
Ending it
You can revoke it at any time by writing to us or from your account. Revoking stops new requests; it can't undo copies already sent.
Your rights
Your providers can't make your treatment, payment, enrollment or eligibility for benefits depend on whether you sign. Records we receive may no longer be protected by HIPAA in our hands, but our privacy policy and the law that applies to us still cover them. You may ask for a copy of what you signed at any time; it is always in your account.
Electronic signature
You agree to sign electronically. Your electronic signature has the same effect as a signature on paper.
